Fees/Method of Payment The initial consultation fee is $150, after which, each subsequent session fee is $120. Fees are to be paid at the conclusion of each session, unless other mutually satisfying arrangements are made. Cash and personal checks are acceptable forms of payment (a receipt will be provided upon request). Minimal phone consultation or correspondence no charge Extensive phone consultation or correspondence more than 15 minutes $120.00 per hour
1st Missed appointment or cancellation of less than 24 hours notice $60.00 Subsequent missed appointments $120.00
Insurance may not reimburse for review of records, extensive phone consultation or missed appointments.
